Who needs a Form CRA-23?
A car driver, got in an accident in Massachusetts, must fill out Motor Vehicle Crash Operator Report (Form CRA-23) in the following cases:
- Anyone was killed or injured in an accident;
- Damage to property caused by the accident amounts to a thousand dollars or more.
If one of the participants of the accident unlawfully left the scene, the police station shall adopt a report, even if the amount of the damage caused is less than $1,000. However, if the accident caused injury to a person or animal, the report’s submission is mandatory.
What is the CRA-23 for?
This motor vehicle crash report describes the type of accident, the parties involved in a car accident, as well as the consequences of a crash. Information taken from the report is used by the competent authorities for legal and financial evaluation of the accident.
Is the CRA-23 accompanied by other forms?
This Massachusetts crash report can be submitted as a separate document without accompaniment. However, medical reports and written testimony of eyewitnesses may accompany this form in case of special need.
When is the CRA-23 due?
The man who was driving the vehicle at the time of the accident, or the owner of the vehicle must submit a report not later than five days after the accident.
How do I fill out Form CRA-23?
Filling out the CRA-23, you must provide information on the following items:

- Information on The Vehicle That Was Driven;
- Other Vehicles Involved in The Crash;
- Non-Motorist(s) Involved;
- Crash Conditions and Diagram;
- Witness information;
- Property Damage Information;
- Description of What Happened.
A submitter should use official names of all locations, streets and landmarks, be as precise as possible and provide enough information to locate the crash to a specific point, not just the street or roadway.
Where do I send Form CRA-23?
One copy of the completed and signed report must be sent to the local police station of the region where car accident occurred. Another copy should be sent to the submitter’s insurance company. And the last one should be directed to the REV at the following address:
